ACCIDENT REPORT

DRIVER INSTRUCTIONS IMMEDIATELY FOLLOWING AN ACCIDENT

  • Pull over in a safe place and activate four-way flashers.
  • Call 911 and share location and basic accident information.
  • Ensure the scene is safe before exiting the cab.
  • Wear high-visibility vest and follow DOT emergency breakdown procedures.
  • Assess the well-being of everyone involved in the accident.
  • Contact the Safety Department to report an accident.
  • Take photos of the scene and vehicles – close-up and wide-view photos. If another tractor is involved, obtain DOT # and Tractor/Trailer #’s.
  • Take photos of the other party’s: a) driver’s license, b) license plate and c) insurance card.
  • Obtain names and telephone numbers of witnesses.
  • Discuss the accident with law enforcement. Do Not discuss fault with other drivers.
  • Obtain a police case number from the responding officer
